We are required to hold a signed questionnaire before we commence your work therefore it is important that we receive completed and signed questionnaires to avoid unnecessary delays.
Please select and complete one questionnaire for each of your entities that require financial statements or income tax returns to be completed by us. Once completed, please send with your supporting records.
Please note, if you choose to email the completed questionnaires back to us, your email and attachments will be kept as the document of record.
Remember to add to your questionnaire answers and/or documents for the following:
- Covid-19 support payments
- Sale and Purchase Agreements and settlement statements for sale and purchase of property
- Finance agreements for new motor vehicles or assets
- Loan statements at balance date
